Scoring notes
The calculator uses the highest score in each site category rather than adding multiple lesion depths in the same site. The uterus surface is counted as peritoneum. Bowel, urinary tract, vaginal, cervical, skin, and other disease should be documented separately because it is not directly scored in rAFS.
This tool is intended for score calculation and documentation support only; final staging should be verified against operative findings and local clinical practice.
Fertility context is intentionally cautious: rAFS stage is a surgical extent score, while fertility counseling usually also needs age, ovarian reserve, tubal function, partner factors, prior pregnancy, and tools such as the Endometriosis Fertility Index.
